Wherever you are on your health journey, our health plans include all kinds of wellness programs and health services designed to support you.

Diabetes Management

The Diabetes Disease Management Program connects you with experts who can provide information to help you manage your diabetes, manage your medications, and improve your diet and exercise.​

The program provides members with personalized 1-on-1 support designed to address their needs throughout the care journey. Our case managers engage with SHBP members a minimum of every 30 days and every 90 days if their Diabetes is well controlled. Additionally, provider outreach is made to obtain clinical updates including notes, labs and other clinical indicators.​

Our enhanced digital capabilities provide the right balance of education, tools and motivation as well as increased communication to help us more closely manage care for SHBP members. SHBP members can exchange messages with their nurse, access curated educational content, and their nurse can also communicate on changes in health data (provided by employee) like blood pressure, weight and glucose.​​

Asthma Disease Management

The Asthma Disease Management program helps reduce unnecessary hospitalizations and health care costs, missed days of work or school and improve the member’s quality of life. The program provides information and resources to help participants:

  • Avoid triggers and recognize warning signs of an attack​
  • Reduce or eliminate risk factors​
  • Follow their medication regimen and treatment plan​
  • Effectively manage their condition and co-morbidities, including depression​​

Chronic Obstructive Pulmonary Disease Program

The Chronic Obstructive Pulmonary Disease Program will help you manage your condition with:​

  • A personal early detection plan to recognize respiratory symptoms​
  • Programs that may help you to reduce risks like smoking and second-hand smoke exposure​
  • Reports that are sent to your doctor if serious issues are detected​
  • Support to help you manage high blood pressure, diabetes, Coronary Artery Disease and other conditions that can affect your COPD​

Congenital heart disease support

Access a team of specialized Congenital Heart Disease nurses that will support you through all stages of treatment and recovery. They will help you make informed treatment decisions and guide you to top-performing Centers of Excellence providers.​

Weight-loss surgery support

Get ongoing support before and after weight-loss surgery from a dedicated team of nurses. Our Bariatric Resource Services program offers educational resources, surgery preparation assistance and continued support to help you succeed.

Mental health support

Calm Health is available through your benefits plan at no additional cost to you. This means you’ll have access to the widely recognized mental well-being Calm app, which offers resources for sleep, meditation and mindfulness With 24/7 access to digital content, you can set your own pace on the path toward a healthier you as you get help with information to meet goals like better sleep, building skills to manage stress, developing resiliency and start building a mindfulness habit.

Guided cancer support

With Cancer Resources Services, you’ll have access to experienced cancer nurses who can provide guidance and support to you and your family during your cancer journey. They’ll help you understand your diagnosis, share information that you help you make more informed decisions about your care and find emotional support for you and your family members.

A cancer diagnosis can bring on a range of emotions and concerns. Providing specialized cancer care management, our Cancer Support Program connects you to a team of highly rated physicians, providers, personal care nurses, and advocates. They’ll help you understand your diagnosis, monitor your medications and treatment plans, provide access to social workers for financial, transportation, and childcare needs, and more.

Help for kidney disease

If you’ve been diagnosed with kidney disease, the Comprehensive Kidney Solution program will connect you with experienced nurses who can help you learn how to manage the disease. They will also work with your doctor, dialysis centers and others on your care team.

Maternity support

Maternity support is designed for all parents, no matter what the pregnancy journey looks like. Providing information and resources on everything from pregnancy planning to postpartum care, this program includes 24/7 access to online courses covering trimester benchmarks, nutrition and exercise during pregnancy, breastfeeding and more.
