Both plans give you access to our large statewide and national network, cover preventive care services 100% in the network (when coded appropriately as preventive care per the Affordable Care Act guidelines), and helps give you financial protection and health care support. Plus, each plan allows you and your covered spouse to reduce your out-of-pocket costs by completing your wellness activities to each earn 480 credits toward your HIA and earn a $250 Reward Card from UnitedHealthcare up to $500 per household.

Access to telehealth and emotional support

On Jan. 1, 2023, the self-help app, Sanvello®, transitioned to Self Care by AbleTo®. Self Care includes many of the same features and benefits as the Sanvello app and members will still have access at no cost.

Existing users of the Premium version of Sanvello may continue to use it throughout their annual subscription were be invited to transition to Self Care by AbleTo beginning in January 2023, or at the renewal of their annual subscription.

We've extended access to telehealth visits to make it easier to connect with health care providers from the safety of your home.

UnitedHealthcare Reward Card and well-being incentive credits

Because achieving your best health is an important personal goal, UnitedHealthcare will reward you and your spouse for the steps you are taking by awarding a $250 Reward Card up to $500 per household when you each complete your wellness activities and redeem your 480 points through the Sharecare Redemption Center.  

Earning the UnitedHealthcare reward is easy. Here's how:

  • When you choose a UnitedHealthcare HMO or HDHP, you and your covered spouse are each eligible to earn up to 480 points to use toward eligible medical and pharmacy expenses by completing the actions under Be Well SHBP.*
  • Once you each complete your wellness activities and redeem your 480 points through the Sharecare Redemption Center, UnitedHealthcare will mail you a $250 Reward Card within 8 weeks. 

To qualify for points with the Be Well SHBP* well-being program and the UnitedHealthcare Reward Card, you must complete your wellness activities and redeem them between January 1, 2024 and December 2, 2024.

*Be Well SHBP is the SHBP well-being program administered by Sharecare.

These services may be based on your age and other health factors. Other routine services may be covered under your plan, and some plans may require copayments, coinsurance or deductibles for these benefits. Always review your benefit plan documents to determine your specific coverage details.
