With Cancer Resources Services, you’ll have access to experienced cancer nurses who can provide guidance and support to you and your family during your cancer journey. They’ll help you understand your diagnosis, share information that you help you make more informed decisions about your care and find emotional support for you and your family members.
A cancer diagnosis can bring on a range of emotions and concerns. Providing specialized cancer care management, our Cancer Support Program connects you to a team of highly rated physicians, providers, personal care nurses, and advocates. They’ll help you understand your diagnosis, monitor your medications and treatment plans, provide access to social workers for financial, transportation, and childcare needs, and more.
If you have a special condition, our Centers of Excellence (COE) network provides help finding a doctor and medical center as well as help with understanding your illness. COEs are intended to complement local hospital and physician care by helping you get access to care.
If you’ve been diagnosed with kidney disease, the Comprehensive Kidney Solution program will connect you with experienced nurses who can help you learn how to manage the disease. They will also work with your doctor, dialysis centers and others on your care team.
When you have a baby in the neonatal intensive care unit (NICU), you want to make sure they’re getting the very best care. Neonatal Resource Services offers a team of skilled neonatologists, NICU nurse case managers and social workers to help during your baby’s stay. They provide support from admission to after discharge, personalizing their care to your baby’s needs.
With Transplant Resources Services, you’ll have access to support throughout your care. In addition to being connected with experienced nurses who will help coordinate care with your doctors, you’ll have access to travel and lodging assistance as well as expert clinical support to help inform you on treatment plan options.
There are things you can do before your plan begins to help prepare for a smoother transition, like checking if your provider is in network, managing your medications and seeing if you qualify for Transition of Care.
