The importance of a primary care provider

Whether or not you elect a medical plan that requires a primary care provider (PCP), one of the most important decisions you’ll make for your health is choosing a PCP. A PCP is the doctor who knows you best.

Your PCP is typically your first point of contact when you have medical questions or issues. They can help you navigate these decisions and get you the care you need. A PCP may also help you save time and avoid surprises.

As your primary contact, they’ll know your health history and health goals. In addition, they provide routine care – such as annual checkups – which may help identify potential health issues earlier. They can refer you to a specialist, if needed. So even when a health plan doesn’t require you to select a PCP, it’s still a good idea to have one.

Transferring records to a new provider

If you’re selecting a new primary care provider, there’s an easy way to make sure your medical history stays with you. To transfer your records, you have 2 options:

  • Complete this release form and send it directly to the office of your current PCP.
  • Complete this release form when you check in for a visit to your new PCP — the office will help ensure your records are transferred.
