  • Provider Network

Our national network of providers  includes a mix of ophthalmologists and optometrists in both private practice and retail settings. Using network eye-health providers will help you save money.

  • Contacts

If you choose contact lenses instead of eyeglasses, this vision plan covers the fitting and evaluation fees, contact lenses (including disposables) and up to two follow-up visits with your eye doctor. If you choose contacts that are not covered in full, you will get an allowance toward the purchase price and we waive the copayment.

  • Children’s Eye Care Program

With this eye care program, children up to age 13 can receive a second eye exam each plan year. The program also offers coverage for a new pair of glasses (frames and lenses) if your child's vision prescription changes .5 diopter or greater in a plan year. This coverage is available at no additional premium cost and standard copays apply.

  • Comprehensive Exam

You get a comprehensive eye exam when you see a network vision provider.
If a disease or eye disorder is found, you'll be referred to your health plan for medical eye coverage. Retinal screening is available to help assess your eye health.

  • Frames and Lenses

When you use a network provider, you have an allowance you can use to help buy any frames your eye doctor offers. High-index lenses, anti-reflective coatings and more are offered at a discount. Standard scratch-resistant coating is included at no additional charge.
