Wherever you are on your health journey, our health plans include all kinds of wellness programs and health services designed to support you.

Access top care facilities

If you have a special condition, our Centers of Excellence (COE) network provides help finding a doctor and medical center as well as help with understanding your illness. COEs are intended to complement local hospital and physician care by helping you get access to care.

Centers of Excellence

The Centers of Excellence (COE) program providers and medical centers are independent contractors who render care and treatment to health plan members. The COE program does not provide direct health care services or practice medicine, and the COE providers and medical centers are solely responsible for medical judgments and related treatments. The COE program is not liable for any act or omission, including negligence, committed by any independent contracted health care professional or medical center.

Maternity support

Maternity support is designed for all parents, no matter what the pregnancy journey looks like. Providing information and resources on everything from pregnancy planning to postpartum care, this program includes 24/7 access to online courses covering trimester benchmarks, nutrition and exercise during pregnancy, breastfeeding and more.

Employers are responsible for ensuring that any wellness programs they offer to their employees comply with applicable state and/or federal law, including, but not limited to, GINA, ADA and HIPAA wellness regulations, which in many circumstances contain maximum incentive threshold limits for all wellness programs combined that are generally limited to 30 percent of the cost of self-only coverage of the lowest-cost plan, as well as obligations for employers to provide certain notices to their employees. Employers should discuss these issues with their own legal counsel.

Weight-loss surgery support

Get ongoing support before and after weight-loss surgery from a dedicated team of nurses. Our Bariatric Resource Services program offers educational resources, surgery preparation assistance and continued support to help you succeed.

Disease Management programs and services may vary on a location-by-location basis and are subject to change with written notice. UnitedHealthcare does not guarantee availability of programs in all service areas and provider participation may vary. Certain items may be excluded from coverage and other requirements or restrictions may apply. If you select a new provider or are assigned to a provider who does not participate in the Disease Management program, your participation in the program will be terminated. Self-Funded or Self-Insured Plans (ASO) covered persons may have an additional premium cost. Please check with your employer.

Coping with cancer

A cancer diagnosis can bring on a range of emotions and concerns. Providing specialized cancer care management, our Cancer Support Program connects you to a team of highly rated physicians, providers, personal care nurses, and advocates. They’ll help you understand your diagnosis, monitor your medications and treatment plans, provide access to social workers for financial, transportation, and childcare needs, and more.

Fertility support

Fertility Solutions offers expert support and resources to help you learn about and explore options for expanding your family. You can schedule a call with a fertility nurse, search for fertility Centers of Excellence, and access 24/7 online educational resources.

Fertility Solutions program should not be used for emergency or urgent care needs. In an emergency, call 911 or go to the nearest emergency room. The information provided through this program is for information only. It is provided as part of your health benefit plan. Program nurses and other representatives cannot diagnose problems or suggest treatment. This program is not a substitute for your doctor’s care. Transplant care support

With Transplant Resources Services, you’ll have access to support throughout your care. In addition to being connected with experienced nurses who will help coordinate care with your doctors, you’ll have access to travel and lodging assistance as well as expert clinical support to help inform you on treatment plan options.
