With this health plan, you can use any doctor, clinic, hospital or health care facility in our network ... which helps save you money. But there is no coverage if you see an out-of-network provider.

You and each covered family member need to select a network Primary Care Provider (PCP) at enrollment.  Please see your plan documents to learn if you need to obtain a referral from your PCP to see a specialist. 

Age-appropriate preventive care services - such as routine wellness exams and immunizations - provided by a network doctor or facility are covered 100%.

With this plan, you'll need to select a primary care provider (PCP) for yourself and each covered family member. Your PCP is your health guide - coordinating your care, helping you avoid cost surprises and supporting you in achieving your best health.

If you don't select a PCP by the deadline, one will be selected for you.  But you can always change your PCP anytime at myuhc.com.
