No saved plans
With this copay plan, there is no deductible. The Surest health plan is designed to show clear, upfront prices (copays) to help you find quality providers and opportunities to save.
From the Surest app or website, see the price for providers and treatment options before making an appointment. Search by symptom like “my head hurts” to generate a list of possible providers and treatment options. View provider locations and prices on a map to help you identify where to get care and where you may save money, including cost-efficient virtual visits.
See how you can save using this interactive demo.
Our comparison chart helps you find your best fit by listing important plan benefits side-by-side.
A smart way to choose care
At Surest, we think having actual choices about who to see for care, where to get it, and how much to pay for it can help you make informed decisions.
With Surest, you have access to the large UnitedHealthcare network of doctors, hospitals and clinics. Prices (copays) are assigned to providers and treatments based on quality, efficiency and overall effectiveness of care.
See if your preferred providers are in our network:
At the end of the demo, you'll find a link and guest access code you can use to log into the Surest site. Look up your providers in the network and see their current copay information.
The Surest app is an information tool to help you find care that fits your health needs and budget. If you’re not sure where to start, the Surest app lets you explore care options and costs when it’s convenient for you – day or night. Simply type in a symptom like “headache.” Accessing the large network of UnitedHealthcare providers, the app will populate a list of doctors and treatment options for you to consider.
You can also see a map view of provider locations and upfront prices (copays) to help you decide where to go. Sometimes just knowing how much care is going to cost – before your visit – may help you feel better about getting care. If you already know who you want to see, just type in the doctor name or specialty to get costs and scheduling details.
We believe a health plan that provides this much clarity gives you a superpower, and you get to decide how to use it.
This plan offers flexible spending account (FSA) options through your employer. With an FSA, you can set aside money to help pay for eligible expenses. You don’t pay taxes on the amount you contribute, which helps save you money.
Flexible spending accounts (FSAs) are administered by UnitedHealthcare and are subject to eligibility and restrictions.
